A more precise analysis for a better basis for decisions

As a urologist, you are often faced with difficult treatment decisions where uncertainty about the aggressiveness of the prostate cancer can affect the patient's treatment and quality of life.

PCAI (Prostate Cancer Aggressiveness Index) offers an accurate and reproducible analysis that clearly grades the aggressiveness of the cancer, giving you and your patient a more reassuring basis for the decision. Unnecessary surgery can be avoided or postponed in cases where active surveillance is possible Surveillance vs. surgery. And conversely, studies have shown that PCAI finds aggressive cancer where the Gleason method gives the all-clear.

Without regulatory risk for the clinic

At present, before IVDR approval Implementation is in place, the service is offered as a second opinion to the urologist in charge What is a second opinion?. This means that you as a urologist can refer your patient to Spearpoint without the clinic carrying any regulatory responsibility. The patient is the one who orders the service; we handle the process and report back to you.

Why PCAI?

Up to 20% of all treatment decisions are assessed as incorrect because of the limitations of Gleason grading Limitations of the Gleason method. PCAI was developed in a study of 25,591 patients followed for up to 23 years, and published in Medical Image Analysis, November 2025 Research & publications.

The method is also robust with respect to different laboratory routines for section thickness, staining and the leading scanner models on the market – a prerequisite for a result to be trusted regardless of where the sample was prepared. This is how the analysis works in detail How PCAI works.
